What to expect on the day of your first visit?
Your initial evaluation will be a one hour appointment with a well trained physical therapist. For your appointment you will need to bring your insurance card and the completed intake paperwork that is available above. Please come a few minutes early if you need to fill out the paperwork.

During your hour appointment, the therapist will perform a comprehensive evaluation of your previous medical history and a thorough physical examination. They will also set goals with you, which can be achieved with their care. Treatment will likely occur on your first visit so please wear proper attire that will allow access to the area in need of treatment, generally workout wear with athletic shoes. After your evaluation, the therapist will discuss the frequency and duration of your visits to the clinic and exercises that should be completed at home.

We will schedule appointments with your busy lifestyle in mind- we are available Monday thru Friday
6:30 AM to 5:30 PM.

Insurance Information
Panther Physical Therapy accepts Private Insurance, Medicare, Auto Insurance, Workers' Compensation, and most PPO's and HMO's.
